AUTHORIZATION FOR USE AND DISCLOSURE OF ARTWORK


I understand that Offline Kid Time, Inc. is a nonprofit corporation and that all donations and proceeds help support the corporation’s mission to broaden its reach and give access to families wanting to participate.  Please note the following consents already provided based on your membership/workshop participation:

  1. All Monthly Activity Packets that are returned are eligible to be posted on our website.  If you do not want your child’s artwork to be posted online, please email mtrinidad@offlinekidtime.com.

  2. We take pictures during our workshops and events to post on our website.  If you do not want your child’s picture to be posted online, please email mtrinidad@offlinekidtime.com.

If you want your child’s artwork to be eligible to be printed on a Greeting Card and Yearbook, we ask that you please provide your consent based on the terms described below:

Without expectation of compensation or other remuneration, now or in the future, I hereby give my consent to Offline Kid Time, Inc. and its respective parents, affiliates, subsidiaries, licensees, successors, and assigns to use my child’s name, artwork and photo (if provided by parent/guardian) as set forth in this Authorization (“Authorization”).

  1. Type of Information to Be Released:  Photograph of child (if provided by parent/guardian), first name, age, town and artwork provided.  Provision of these items do not involve any direct or indirect compensation/payment from Offline Kid Time, Inc. or any third party for its use.

  2. Copyright: To authorize Offline Kid Time, Inc. to copyright publication of any assets created by Offline Kid Time, Inc. in the form of either the Yearbook or Greeting Cards.

  3. Purpose of the Request: 

    • Yearbook - To compile, create and sell member’s work at the end of each year to capture their progress and cherish the memories.  The children can read and look through their peer’s artwork and written work.

    • Greeting Cards - To create and sell greeting cards featuring children’s artwork.

  4. Persons Authorized to Receive Information:  All personal information provided to Offline Kid Time, Inc. is kept confidential and is not sold to third parties. 

  5. Right to Revoke Authorization:  I understand that I am not required to sign this Authorization, and have the right to revoke this Authorization at any time. A revocation of Authorization will not apply to materials, artwork or other information already used or disclosed in reliance upon this document.
